Yes, in Ontario, Canada, you typically need a requisition from a healthcare provider to undergo blood work. This requisition outlines the specific tests that need to be conducted and provides essential information for the laboratory to process the samples accurately. It ensures that the tests are ordered based on medical necessity and align with the individual's healthcare needs. Without a requisition, most laboratories will not perform blood work. To obtain a lab requisition schedule an appointment with a healthcare provider. During the appointment, discuss your health concerns and symptoms. The healthcare provider will assess your medical history and discuss any relevant factors that may warrant tests.
Requisitions are valid for 6 months from the date of issue. They are also for one time use only.
Our healthcare providers can order diagnostic imaging such as ultrasounds, x-rays, and lab work for everything from iron, blood sugar and cholesterol levels to confirm a diagnosis. Please note that lab requisitions are issued based on your specific case and at the healthcare provider’s discretion.
To get your test results you can book a follow up appointment. Alternatively, our clinic will also reach out to you once the test results arrive to schedule your consultation with our healthcare provider.
For Treatment of Patients Residing Outside of Canada
The Patient agrees that the relationship between himself / herself, doctors and healthcare providers of MedCare Connect shall be governed by and construed in accordance with the laws acknowledges that the treatment/services was performed in the Province of Ontario and the Courts of the Province of Ontario shall have jurisdiction to entertain any compliant, demand, claim or cause of action whether based on alleged breach of contract or alleged negligence arising out of the treatment. The patient hereby agrees that he / she will commence any such legal proceedings in the Province of Ontario and only in the Province of Ontario and hereby submits to the jurisdiction of the Courts of the Province of Ontario.
